The efficacy of an octreotide test dose to predict the outcome of treatment for acromegaly with somatostatin LAR

Claridge A , Shakoor S , Trainer P

In many centres a test dose (TD) of octreotide is administered prior to commencing LAR. The value of the TD as a predictor of subsequent response to therapy remains uncertain. We have studied the relationship of the GH response to a TD to the subsequent response to LAR in 26 patients (median age 59 years, range 20- 84). After baseline GH sampling, 50mcg subcutaneous octreotide was administered and sampling continued for six hours. Results of the TD were compared to the lowest ...

A case of Carney complex with unusual presentation

Shakoor S , Lalloo F , Davis J

Carney complex consists of endocrine, cardiac, cutaneous and neural tumours, as well as pigmented lesions in the skin and mucosae. Endocrine manifestations include primary pigmented nodular adrenocortical disease, acromegaly, prolactinoma, large cell calcifying Sertoli cell tumour in males, and thyroid nodules and carcinomas. Here, we report a new case in whom the diagnosis was obscured by complicating presentations.A female patient, presented with seizure like attacks at ...

Reproducibility of cortisol day curve in monitoring patients on long-term Hydrocortisone Replacement Therapy

Shakoor S , Roberts M , Shalet S , Trainer P

The risks of over and under-replacement with glucocorticoid replacement therapy are well-known. In an attempt to minimise these risks cortisol day curves (CDC) are performed in many centres, however there is a dearth of data on the reproducibility of this assessment and the relationship between symptoms and serum cortisol levels. To address these issues, we have performed 2 cortisol day curves within 7 days in 20 patients (9 male, mean age 54 years, range 25-79, 6 primary and ...
